I agree with alert levels 2 and 3, but the presence of hands is not necessary. You just need to make a small 1° steering movement to deactivate the alert, which will reappear 15 seconds later.😓
I do a lot of one hand, straight line interstate driving and rarely get a hands off alert.

I did get more hands off alerts before I installed rear cradle/carrier lockouts, control arms and got a wheel alignment.

Can also use the knee for a quick steering wheel movement, so you don't need to take your hands off your mobile when playing Fruit Ninja.

Regarding the intended position of hands on the steering wheel I strongly believe when designing the airbag behavior they assume you have your hands where the wheel has its support for the thumbs, so that would be 3-9.
I always wonder if those gangster thugs who put their left hand on the 1 position ever thought about how funny their arm will look like when broken in two peaces by their own head.

He's the Ford WorkShop Manual description of the Hands Off Warning ...

Hands-Off Warning
When the lane keeping aid is active, the PSCM continuously monitors the torque sensor in the EPAS system to determine if
a hand is on the steering wheel and sends the data to the IPMA.
So whar triggers the "pull over and rest" warning ?

The hands-off warning is generated in 2 levels. The first level is a message center warning only and is triggered after 3
seconds of hands-off driving is detected and a lane keeping aid intervention occurs.

The second level warning consists of a
message center warning and an audio chime and is generated after 6 seconds of hands-off driving.

NOTE: Due to certain road conditions and the driver's individual grip/touch on the steering wheel, the system may
generate a hands-off warning when hand(s) are still on the steering wheel.
This looks very much like one module is not ‘talking’ to the other. Think about this, if you’re going down a straight stretch of road; I10 east of Houston, TX130, and many more, the camera ‘sees’ the road is straight and that steering input will be minimum, so it’s basically generating false alerts on straightaways. I realize it’s probably the only way to determine if someone is holding the wheel, but there should be a relaxation logic built in when the camera “sees” a straight road.

Torque sensors are used in many cars (including Tesla), it's just the tune used by Ford that doesn't work properly, I hope they will do something in a future update.
Oh.. and today I found my lane assistant back on.. grrr.

Mine does it ALL the time. Straights. Curves. With and without my hands on the wheel. So much that I turn it off.
Mine does it occasionally, it’s no big deal.
After I put my hand back on the wheel. It goes off, immediately.

I’ve probably gone at least 5,000 miles of “hands off” driving with the active lane keep assist (only available on Automatic and 401A). As a matter of fact, it was the major selling point of the car.
it is slightly annoying to have to bop the wheel when driving in a straight line with or without cruise turned on.
HOWEVER, Ford did a great job with the lane assist and like I said, I usually let the car drive itself, especially in traffic. On the highway, off the highway, curvy road or straight road, it performs exceedingly well. Even in the rain or low visibility conditions or poorly marked lanes it does a great job. I’ll give it a little tug every now and then to keep it happy, but I’m essentially driving a Tesla with a 5.0 and a real exhaust note :)
Now, if anyone ever figures out how to mod the timeout warning to go longer between the “keep hands on wheel” messages, I would pay handsomely for the the How-To

Interesting, Personally I find the Lane Assist function is incapable of self driving, I've tried letting it do it';s thing and it just starts oscillating between the lines and just runs off the road in short a time.

Another Update, There was no traffic on my home from werk last night so I decided to see how long it took to trigger the DUI Alert.

With the cruise set on 75 it took 2miles constant weaving to set it.

Do you have "Lane Keeping" on? What that on, the car holds dead center of the lane... on straights, through curves...

With "Lane Assist" on, it'll bounce back and forth between the lines.

"Lane Keeping" is awesome... except for the stupid warning. Tried it again going to work this morning... every 15-20 seconds, even with my hands on the steering wheel.

I confirm lane keeping is awesome and works very well (keeping you in the center and anticipating the road shape), even when the road marks aren't that good, it works a way better than the S550, it's a shame I had to turn it off because of that bugged "keep your hands on wheel" message popping up every 30 seconds. I hope we'll find a way to turn it off via ForScan or tuned properly with a future Ford update.

This exactly. Not sure how some are using lane keeping and not getting the message. Today it was chilly (for FL) so I was fully taking advantage of the heated steering wheel. Still.... stupid warning. Well, let me rephrase - it isn't a stupid message if your hands are off the wheel. It's a stupid message to get EVER if your hands are on the wheel.
